INTAKE FORM

 
 

Before you arrive at your first session, it is important to email me a completed intake form (one per person being counseled). Please download the form by clicking on the button below. You can fill it out with a PDF-writer, or you can print it and fill it out by hand. Please email it to steve@crosstowncounseling.org in the form of PDF, a scanned document, or a set of photos of each completed page.

 
 

 
 

OFFICE LOCATION

 
 

My office is located at 470 Olde Worthington Rd. Suite 200 Westerville, OH 43082. If you arrive at the building between 9am-5pm Monday-Friday, the building will be open. Upon entering, take the elevator to the 2nd floor. Once you step off the elevator, have a seat in the lobby and text me at 212.537.6502 to let me know that you have arrived. I will come get you. If your session begins outside the aforementioned hours, the building will be locked. Please text me when you arrive at the building, and I will come down and let you in.

 
 
 
 
 

In addition to in-person counseling, I also do virtual counseling via Zoom or FaceTime. I counsel people all over the world, and I would love to counsel you no matter where you may live.

 
 

 
 

FEES

 
 

My fee is $195 for an 80-minute session. While this may sound expensive, I assure you that it is well below the market rate for counseling. Sessions are typically held every other week to help mitigate costs, with homework given between sessions. I accept credit cards, Venmo, or Zelle. 

 
 

 
 

FINANCIAL SUPPORT

 
 

If the price of counseling is significantly beyond your budget, I encourage you to contact your pastor, the head of your diaconate, or the person in charge of your church’s mercy ministry to see if funding is available to help cover the cost (it usually is). In very rare occasions, I also offer a sliding scale depending on your financial need.

 
 

 
 

INSURANCE

 
 

While I do not bill insurance directly, I can provide a receipt for you to submit an out-of-network claim. If you are wondering why I do not bill insurance, please read this article that I published with the Biblical Counseling Coalition:
